Transfer Out - Records From Eden Park
We are so sorry to see your family go! We wish you the best and want to make the process as streamlined as possible.
When you have chosen a new provider, download, complete, and provide us with a signed Medical Records Release Form along with $5 per medical record ($5/child). If the patient is over the age of 18, no fee is charged.
Your request to have your medical records transferred will be completed by our Medical Records department within 5 business days via fax. A paper or digital copy of the records may also be requested.
