Missed/Late Cancelled appointments represent a cost to us and to other patients who could have been seen in the time set aside for you. If you need to cancel your well/routine appointment, please contact us at least 24 hours in advance. If you need to cancel your sick/urgent appointment, please contact us at least 2 hours in advance. Recognizing that “life happens” we do not assess a fee for the first missed appointment.* However, a $50 fee will be charged for a second missed appointment. A third missed appointment will result in discharge from the practice.
*New patients who miss their first appointment will not be rescheduled.

Financial Responsibilities:
Eden Park Pediatrics will verify that your insurance on record is active, however, it is the guarantor's responsibility to contact the insurance company to confirm what benefits are covered.
Your insurance policy is a contract between you and your insurance company, even if your employer provides it. There are many subtle differences in insurance policies, and employers frequently change coverage and co-payments. You are responsible for knowing what services are covered, how often (example: well visits for some plans can be scheduled once per calendar year while other plans can only be scheduled a full 365 days from the previous well visit for children over 3yrs old). Some expenses that may be associated with your child’s visit are:
You must present your insurance cards at every visit:
Your insurance card has information you want us to have – like where to send claims, lab work, and who we may refer to. You should also be aware of where your insurance wants you to go for any lab or radiology procedures, so that in an urgent situation, you are seen at the appropriate facility and will not receive a bill.
You must pay your copay at the time of the office visit:
Our contracts with insurance companies require us to collect your copay at the time of service. We accept cash, credit cards, and checks as forms of payment. If you do not pay your copay at time of service, we will assess a missed copay fee of $20.
If your insurance plan requires you to choose a primary care provider, you must contact your carrier and select one of our doctors before your visit.

Well Visits
Regular well-child care is one of the most important steps you can take toward protecting your child from serious illness. Preventive care promotes healthy growth and development. Well-child visits also provide you with the opportunity to discuss any questions or concerns you may have about your child’s health.
Most health plans cover a set of preventive services for children without charging you a copayment or coinsurance. This set typically includes the cost of the well child visit, most immunizations, and many preventive screenings. We follow the guidelines provided by the American Academy of Pediatrics and Bright Futures (https://downloads.aap.org/AAP/PDF/periodicity_schedule.pdf). While most plans cover most services, some plans do not cover some services. You are responsible for understanding your health plan’s coverage and for payment of any non-covered service. Well Visits with Additional Services
When your child is seen for a well- child visit there could be a reason(s) for additional services; these services may not be considered preventive. If a concern needs to be addressed, or you would like to address a problem unrelated to the well visit, the physician will need to provide additional services to care for your child. These additional services are billed to your health plan in conjunction with the preventive services provided on that day. Some services that may be provided and billed in addition to preventive services include:
Our physicians are required to report all services rendered to your health plan based on federal guidelines and the actual services provided. Please feel free to ask questions about services that may not be covered as preventative by your health plan on the day of your visit.

Form Fees -
School/sports/daycare/camp forms will be completed free of charge at the time of a well visit. Please complete all of the parental sections of forms prior to presenting to our staff.
$30 Returned Check Fee -
In the event a personal check is returned unpaid from your bank, your account will be charged with a returned check fee of $30, and your account may be placed on a “cash only” basis until the balance and fee are paid.
$20 Rebilling Fee –

$5 Medical Record Summary Transfer/Copy Fee –
A $5 fee is charged to transfer/produce a copy of your child’s medical record including immunizations, problem list, diagnosis history and growth charts.
$35 After Hours Fee Evening/Saturday/Holiday (Office open) –
For your convenience, we offer appointments after the standard 9 – 5 weekday. We charge a $35 after hours fee for this service. Both insurance plans and parents realize that this fee is a cost-effective alternative to a visit to the Emergency Room/Urgent Care and/or taking time off work/school. This fee is added to all visits that are provided:
$35 After Hours Fee (Office closed)–
If your child needs care when our office is closed, call us. One of our physicians is always on call and will open the office to see your child if it is determined an office visit is advisable. A $35 fee is charged for providing this after hours care.
The best way we can deliver exceptional care to your family is when you are aware of the expenses that are part of your child’s healthcare. We are an independent private practice and a small business, which helps us have an exceptional team of talented healthcare professionals who truly care about your child. We are happy to complete your child's form, but please understand we process a high volume of forms, usually between June and August, and they take time to process with your provider.
We Don't "Just Sign" Forms
Our nursing team reviews your child's medical record and completes part of the form. Next, one of our doctors or nurse practitioners reviews your child's medical record, determines whether your child is healthy enough to be able to participate in the desired activity, completes the remainder of the form, and then signs it.

We are happy to complete your child's School/Daycare/Camp form free of charge at your well child visit. If you think your child may be interested in playing sports, please bring a Sports Form to the physical. Be sure to complete your portion of the form first.

If you need a form completed and your child is not up to date on his/her physical, you will need to schedule the physical. We will complete the form at that time.
Between Well Child Checks?
If you need a form completed and your child is not due for his/her physical, we are happy to do that too. To streamline the process for you, please follow these steps:
Complete your portion of the form.
Provide us with the form, your payment, and desired pickup date. (The form can be uploaded to your portal account, My Kid's Chart, or FAXed, or dropped off at our office.
We will upload the form to your portal account, My Kid's Chart and notify you that the form is complete.

How do I know my credit card information is safe?
Your credit card information is encrypted and stored off-site with one of the nation's top payment gateways. No one at Eden Park Pediatrics has access to your information, and after you have completed the authorization form, you’ll never have to give us that card again. We simply access your account on the gateway and process your payment.
Why should I sign up for Eden Park's EasyPay?
Using Eden Park's EasyPay will make your appointments a breeze! If you use EasyPay you can simply ask staff at check-in/check-out to charge the card on file for co-pays or balances on your account.
As a perk of using EasyPay, if you accidentally miss your co-pay after you leave your appointment, we can charge your account and will waive the $20 missed co-payment fee.
If you have a balance of under $200 and it is over 60 days past due, we can charge the card you've previously authorized and you will not be charged any additional fees.
How do I know how much I had to pay?
As soon as we process a payment, whether it’s a co-pay or a balance due, we’ll immediately send you a receipt via email. You’ll also still receive your Explanation of Benefits from your insurance company.
What if I have a large balance? Will you charge it all at once?
We will speak to you about any balance you have and how you would like to take care of it before we will use your card with EasyPay. If you approve the amount and want to pay it all at once, we’ll process your EasyPay as usual. If you can’t or don’t want to pay it all at once, we’ll work with you to set up a payment plan. It will always be your choice.
What if you make a mistake and charge me too much?
If you get your email receipt and think we made a mistake, just give us a call in the office. Our Billing Specialists are always happy to talk to you about your charges and payments. If we made a mistake, we’ll correct it and refund you right away.
What happens if the card gets charged with insufficient funds?
If we are unable to collect the balance/co-pay from your EasyPay, our billing specialist will reach out to coordinate payment with you.
What if I don’t want to give you my credit card information?
We think the ease and convenience of Eden Park's EasyPay are so great that most of our families will be thrilled to sign up for it, but if you don’t want to, you certainly don’t have to. You can continue to receive traditional paper billing statements. We will mark you as "opted-out" and you will no longer receive texts or promotions to sign-up for EasyPay.
Does Eden Park's EasyPay work with Health Savings Account cards?
Absolutely! It works exactly the same way as it does with other credit cards.

Efficiency through the use of technology
You will be encouraged to consult our website, register for and use our patient portal, and effectively use automated reminders for appointments and for routine care/immunizations that are due.
Eden Park Pediatric Associates is committed to providing our patients the best medical care possible. We believe that to do this effectively we must enter into a partnership based on mutual trust with the parents of our patients.
Eden Park Pediatrics strongly believes that the current vaccine schedule is the safest and the most effective way to immunize your child. If you refuse to follow the current vaccine schedule, we will ask you to find another health care provider who shares your views. Refusal to vaccinate your child per the recommended guidelines places your child at a serious health risk. We simply cannot do that.
We recognize that the choice to vaccinate may be a very emotional one for some parents. We will do everything we can to reassure you that vaccinating according to the schedule is the right thing to do. We are happy to discuss your concerns about vaccines at your child's next visit, but we will not alter this policy.

Our Immunization Philosophy
Our practice believes that all children should receive the recommended vaccines according to the guidelines provided by the AAP and the CDC. Vaccines are safe and effective in preventing diseases and health complications in children and young adults. Regular vaccinations help children ward off infections, and are administered as one of the safest and best methods of disease prevention.
As medical professionals, we feel very strongly that vaccinating all children on schedule with currently available vaccines is absolutely the right and responsible thing to do for all children. We believe that immunizations are one of the most important health interventions a parent can do on behalf of their children.
Eden Park Pediatric Associates follows the recommended childhood immunization schedule of the American Academy of Pediatrics and the American Committee on Immunization Practice (ACIP). We feel the current immunization schedule provides the best medical care to prevent these diseases that can cause severe illness, brain damage and death. The current immunization schedule has been thoroughly researched and has been repeatedly found to be safe and to be the best way to prevent these illnesses in young children. Delaying these immunizations puts your child at risk for developing these illnesses and does not decrease or otherwise alter the mild side effects sometimes seen from these immunizations. The delay or refusal of vaccines has caused resurgence of these diseases worldwide.
We firmly believe that the current vaccines are safe and effective. Vaccines do not cause autism or developmental disabilities. Thimerosal is still a preservative in a few vaccines and has been shown not to cause autism or developmental disabilities nor an elevated mercury level in those patients who have received thimerosal containing vaccines. The previous concerns about the association of the MMR vaccine and autism has been refuted and multiple studies have shown this to be a safe and effective vaccine. Separating vaccines to give over additional visits goes against expert recommendations and is not supported by any scientific data.
